The Problem

Research studies have shown that older adults have a higher quality of life and more happiness when they remain living in their own home. Older adults have higher rates of depression when being placed in an institution, and the medical care is seemingly inadequate. The current home care agency model is outdated and does not provide sufficient healthcare for an older adult who wants to remain healthy at home.

MIND & MOBILITY HOME CARE & THERAPY CENTERS METHOD Our therapists and caregivers use a biopsychosocial approach which focuses on maximizing independence, preserving cognitive and physical function.

COMMUNITY

The exercise centers at MIND & MOBILITY HOME CARE & THERAPY CENTERS offer an engaging community to our members. Our therapists and caregivers work together at our exercise centers to facilitate programs that address our member’s social, emotional, cognitive, functional mobility and balance needs.

MOBILITY

MIND & MOBILITY HOME CARE & THERAPY CENTERS offers UNLIMITED access to THE CENTERS. Every home care member receives one-on-one sessions with a licensed physical therapist and occupational therapist to work on balance, functional strengthening and gait training exercises.

MIND

Our cognitive therapeutic programs include evidence-based activities designed for individuals with memory impairment and a combination of physical and cognitive stimulation exercises.

HOME CARE

Members are given the opportunity to select their own caregiver. Caregivers can escort members to the exercise centers, where rehabilitation professionals and the caregiver provide one-on-one therapy. This interdisciplinary approach ensures maximum safety in the home and best care outcomes.

MIND & MOBILITY caregivers offer:

  • Cooking
  • Light Housekeeping
  • Grocery shopping and errands
  • Companionship
  • Assistance with bathing, dressing, and grooming
  • Status reporting to family
  • Controlling Infections
  • Transportation to medical appointments, supermarket, and pharmacy
  • Assistance with walking and transfer from bed or wheelchair
  • Assistance with medication reminders

CARE MANAGEMENT

Our care management services, which are coordinated by licensed clinical social workers, provide resources needed to allow members to continue to live at home. Counseling services for our members and their families are also offered.

Our registered nurses are trained to provide our members with the following services:

  • Brain and spinal cord injury care

  • Implementing a clinical care plan
  • Coordinating home medical equipment, pharmacy, & supplies
  • In-home physical, occupational and speech therapy

MIND & MOBILITY MEMBERSHIP

MIND & MOBILITY HOME CARE & THERAPY CENTERS services are covered by medical insurances, long term care policies and private pay. Members are given the opportunity to select their own caregiver, and memberships include unlimited physical and occupational therapy, regular registered nurse visits, and care management from a licensed clinical social worker. The hourly cost starts from $14 per hour for hourly care. Live-in 24-hour care starts from $150 per day.
